The program is a comprehensive system-of-care that addresses nutritional needs, physical and mental health, financial well-being, and essential community-based services. The program also ensures access to reputable referrals and resources, including support for end-of-life planning when requested by families or primary caregivers. Nutrition education and chronic disease wellness classes are interwoven into the events, as well as, cultural activities and annual cultural & holiday celebrations, and field trips.
Our Edler Care Team members maintain meaningful engagement throughout each participant’s life journey, (including hospital visitations and rehabilitation stays, attendance at funeral services, and thoughtful support for repast gatherings), honoring the full continuum of care with integrity and compassion.
